Forms and Applications

Please note: If opening a form with a browser other than Internet Explorer you may encounter difficulty in completing the forms electronically. If you are having difficulty filling out a form, please follow the steps below which explain how to save the file to your computer.

  3. Select a location on your computer to save the file.
  4. Open the file from the saved location.

  5. Complete the form.

Note: The new Form 2 must be used for all plan years ending on or after January 1, 2014 though earlier adoption is permitted.
